Patient Portal

Your patient portal is the easiest way to manage your care between visits. Through the portal, you can request prescription refills, view your visit summaries, and access lab and test results.

If this is your first time accessing the portal, watch for an invitation email from AllergyMD with setup instructions. Contact our office if you need help.

New patient forms

Most new patients complete intake forms online before their visit through our patient portal — it saves time and lets us prepare for your appointment. If you'd prefer to fill out printed forms, we will provide a copy for you upon arrival. Please arrive 30 minutes early to allow time to complete and to verify your insurance.

Bill Payment

AllergyMD makes it easy for our patients to pay for their medical care by offering secure, online bill payment via Healow Pay. Simply click on the "Pay My Bill" button below to start the process.

Please use the Statement Code on your most recent statement. If you have any questions, please contact our office.

Office Policies

Appointment cancellations

We understand things come up. We ask for at least 48 hours notice if you need to cancel or reschedule. Cancellations with less than 48 hours notice may incur a $50 fee.

To cancel or reschedule, click the link in your appointment reminder text or email, or contact our office.

Prescription refills

To request a refill for medications previously prescribed by AllergyMD within the past year:

  1. Text our office at 678-632-2360

  2. Request the refill at your next appointment

Please allow up to one week for refill requests. Refills require an active patient relationship — if it's been more than a year since your last visit, you'll need to schedule an appointment before we can refill.

Insurance and referrals

We accept most major insurance plans. Visit our insurance page for a current list and verification guidance. If your plan requires a specialist referral, please obtain it from your primary care provider before your appointment.

Billing and payment

We bill your insurance directly as a courtesy. Patients are responsible for copays at the time of service, and any portion not covered by insurance.

We request that all patients keep a credit card on file. If you'd prefer not to, you'll need to pay your estimated balance at each visit. We accept Visa, Mastercard, and Discover.

If you're uninsured or out-of-network, contact our office to discuss self-pay options. We can provide a Good Faith Estimate as required under the No Surprises Act.

Visitor and food policy

Please limit visitors during your appointment to minimize exposure to others. Masks are not required but are welcome — if you'd prefer Dr. Lee to wear one, just let us know upon arrival.

Out of consideration for patients with food allergies, please do not bring outside food into the office unless we've specifically asked you to.

Frequently Asked Questions

  • Your first visit typically takes 30-60 minutes. Dr. Lee will review your medical history, examine you, and consider allergy testing if appropriate. Most patients leave with a clear diagnosis and a personalized treatment plan in hand.

  • Most insurance plans don't require one, but a few do. Check our insurance page for details, or call our office and we'll help you verify.

  • If you're scheduled for skin testing, stop oral antihistamines (Zyrtec, Allegra, Claritin, Benadryl) seven days before your visit. Continue all other medications unless we tell you otherwise. Bring a list of your current medications, including over-the-counter products and supplements.

  • Yes. Dr. Lee is dual-trained in Internal Medicine and Pediatrics and treats patients of all ages. Many of our families schedule appointments together when possible.

  • Please call or text 678-632-2360 to let us know. We'll do our best to accommodate you, though depending on the day's schedule we may need to reschedule if you'll arrive significantly late.

  • Complete this medical records release / request through the patient portal. We typically process records requests within 3 business days. Please contact our office with questions or if you need it sooner.

  • You can pay through the patient portal or by calling the office during business hours. We accept Visa, Mastercard, and Discover. Bills not paid within 30 days incur a $35 late charge; balances unpaid 90 days from the explanation of benefits date may be sent to collections.
